This guide provides information to assist Indigenous applicants to prepare and submit their grant applications for the Indigenous Primary Health Care Innovation Fund. The Innovation Fund provides funding to support Indigenous communities and organizations in pursuing priorities based upon their unique primary health care needs. The intent is to lay a solid foundation for the delivery of accessible, relevant, high-quality, and culturally-safe primary health care led by First Nations, Métis, and Inuit peoples in Alberta.

The Swan Hills Treatment Centre (SHTC) is a facility for the safe disposal of special wastes located approximately 12 km northeast of the town of Swan Hills, AB. On October 16, 1996, a malfunction of a transformer furnace was discovered to have caused the release of a portion of process gases containing polychlorinated biphenyls (PCBs), polychlorinated dibenzo-p-dioxins and dibenzofurans (PCDD/Fs) into the ambient air. These chemicals can be harmful to human health. Therefore, in 1997 Alberta Health (AH) conducted a human health risk assessment study to estimate human exposure and to evaluate the effectiveness of public health interventions. As a result of the study, food consumption advice for wild game and fish taken within a 30 km radius of the facility were issued out of concern that PCBs and PCDD/Fs were accumulating in these locally caught foods. This is the final report of the Indigenous Primary Health Care Advisory Panel, which was established as part of the Modernization of Alberta's Primary Health Care System initiative to provide advice and recommendations to help transform Alberta's primary health care system in order to provide accessible, relevant and culturally safe primary health care for Indigenous peoples. It contains 22 overarching recommendations organized around five strategic outcomes of primary care: culturally safe and appropriate care, access, integration, quality, and Indigenous peoples as partners.

The purpose of the forum was to discuss current challenges in Alberta's primary health care system for First Nations, Métis and Inuit peoples, and to identify innovative solutions from other jurisdictions to address these challenges. Results of the forum will also inform inform the Indigenous Primary Health Care Advisory Panel's final report to the Minister of Health.

The Continuing Care Capital Program provides capital grant funding program to increase continuing care spaces in Alberta. The Indigenous stream of the Program supports the development of continuing care spaces for Indigenous peoples in Alberta, increasing continuing care capacity for Indigenous populations on or off reserves or on or off Metis Settlements. Documents in this package include an outline the application process, a guide with information to assist applicants in the preparation and completion of the business case application form, and the application form itself. The deadline for submissions was December 17, 2021.
