Alberta Health and Wellness undertook a review of laparoscopic adjustable gastric banding (LAGB) in late 2004 and early 2005. A synthesis report was forwarded to selected stakeholders in June 2005 for response. There was considerable support expressed among stakeholders for offering the LAGB device as a medically insured good.

Description

A review of the laparoscopic adjustable gastric banding (LAGB) device was undertaken in response to continued requests from Albertans for access to this surgery for treatment of clinically severe obesity. The review was undertaken to provide government with appropriate evidence and information to make a decision on its public funding. Its three specific objectives, representing the range of information required; were to review: the effectiveness and safety of LAGB; the social and economic considerations for the provision of LAGB; and the fiscal considerations for the provision of LAGB.

Description

Clinically severe obesity is associated with a range of co-morbidities such as type 2 diabetes, hypertension, dyslipidemia, sleep apnea, and increased risk for cardiovascular diseases. Conventional treatments such as diet and exercise, behavioural modification regimens, and pharmacological interventions have been shown to be ineffective for this group of patients. Bariatric surgical procedures are considered as the last-resort therapy for severe obesity, but their long-term safety and clinical efficacy remains to be determined. The objective of this study was to determine whether laparoscopic adjustable gastric banding (LAGB) is a safe and effective procedure compared with open and/or laparoscopic Roux-en-Y gastric bypass and laparoscopic vertical banded gastroplasty, especially in the longer term, for adult patients with clinically severe obesity.
