Presents the findings of an investigation into a workplace fatality in the transportation communication and utilities sector that occurred in February, 2008. A 55-year old male worker entered a pup trailer that had contained produced water and had just been steam cleaned. The worker entered the vessel with a trouble light to check a crack in one of the baffles when an explosion occurred.
