The objective of this health evidence review was to conduct clinical reviews and an economic evaluation to address the following overarching policy question: What is the optimal strategy or set of strategies in the delivery of high-quality and cost-effective stroke rehabilitation care in Alberta, subject to geographical constraints and justifiably consistent with the Canadian Stroke Best Practice Recommendations for stroke rehabilitation?

Description

The primary objective of this health evidence review was to conduct a clinical review and economic analysis of currently existing vascular risk reduction initiatives, contextualized to the Alberta setting and considering patient views regarding vascular disease prevention. At the time of this study, vascular diseases (which include cardiovascular disease, stroke, type 2 diabetes mellitus, chronic kidney disease, and peripheral vascular disease) affected the lives of more than 300,000 people in Alberta.

The objective of this report is to provide a synthesis of evidence regarding currently existing urgent care models and their effectiveness, with a focus on the following research questions: What models of delivering urgent care exist in Canada and other select countries with similar health systems? And how safe, effective, and economical are these urgent care models, and what factors contribute to the safety and/or effectiveness? In general, urgent care is defined as unscheduled health care offered, as part of emergency department care or in ambulatory settings, for acute lower-acuity conditions.

The clinical review and economic evaluation aimed to determine how Oncotype DX and Prosigna can be optimally used to determine which patients with early-stage breast cancer will benefit from adjuvant chemotherapy. This report addresses the following research question: For patients with early-stage (I–III), ER+, HER2−, node-negative or node-positive (one to three nodes) breast cancer, what are the clinical and economic benefits of Oncotype DX and Prosigna genetic testing, and how do these differ by node status, risk status, age, and menopausal status?

It also describes the nature varicose veins with venous reflux, the available clinical practice guidelines regarding the management of the condition, as well as the technologies currently being delivered in Alberta to treat the condition and the available system capacity for delivery of the technologies being assessed.

Preterm birth (PTB) is one of the biggest problems in modern obstetrics, as it is responsible for the majority of perinatal mortality and adverse long-term neurological outcomes. For spontaneous preterm labour (PTL) without rupture of membranes, the majority of women presenting with symptoms will go on to deliver at term. Thus, the challenge in practise is to correctly identify those women who are in true preterm labour and will deliver preterm. The fetal fibronectin (fFN) test is a diagnostic test that can be used to help diagnose PTL in women. The primary advantage of the test is its simplicity of use and the strong negative predictive value. This potentially gives the ability to more accurately identify those women who are in true preterm labour, and who therefore need to be transferred and treated. On April 25, 2006 the Assistant Deputy Minister, Program Services Division of Alberta Health and Wellness issued a request for feedback on fetal fibronectin testing (fFN) for the diagnosis of preterm labour. The consultation was based on work undertaken through the Alberta Health Technology Decision Process culminating in the production of a Synthesis Report. The Synthesis Report was distributed with the invitation letter and consultation questions (Appendix A). The Synthesis Report was based, in large part, on work undertaken through the Alberta Heritage Foundation for Medical Research Health Technology Unit and the Institute of Health Economics.
