A worker was preparing to weld a pipe inside of a trench. Another pipe located beside the trench fell into the trench crushing the worker. Presents the findings of an investigation into a workplace fatality in the mining and petroleum development sector that occurred in March, 2007. A male worker, employed as a pipe layer, was driving east on a haul road. As the worker approached a single-lane bridge, a log truck was also approaching the bridge from the opposite direction. The worker drove his truck passenger side wheels into a snow bank, the driver’s side of the truck caught the side rail of the bridge and flipped over landing on its roof in the river. The worker was partially ejected from the vehicle and his head struck a rock on the bank of the river.
