Worker fatally injured in vehicle incident

Description

Presents the findings of an investigation into a workplace fatality in the mining and petroleum development sector that occurred in March, 2007. A male worker, employed as a pipe layer, was driving east on a haul road. As the worker approached a single-lane bridge, a log truck was also approaching the bridge from the opposite direction. The worker drove his truck passenger side wheels into a snow bank, the driver’s side of the truck caught the side rail of the bridge and flipped over landing on its roof in the river. The worker was partially ejected from the vehicle and his head struck a rock on the bank of the river. See the usage considerations section of this record for more information about OHS work site investigations.

Usage Considerations

Alberta Occupational Health and Safety (OHS) investigates serious incidents, including fatalities, which occur at work sites that fall under provincial OHS legislation. After a serious incident or fatality takes place at a work site, OHS officers investigate the work site to determine the causes and circumstances of the incident or fatality. OHS prepares an investigation report summarizing the incident and then conducts an enforcement action review of the investigation, to determine if the incident file should be submitted to Alberta Justice for consideration for prosecution. OHS publishes the fatality investigation report online only after investigations and all court proceedings, if any, are completed.
