From April to December 2021, there were 36 deaths of children, youth and young adults receiving services from the ministry of Children's Services. The Minister of Children's Services directed the ministry to carry out a review of the cases to gain an understanding of the circumstances surrounding these deaths, to help identify improvements that could be made to better serve children, youth and families. This publication reports on the findings of three separate reviews undertaken by the ministry, and actions that will be taken by the ministry in light of these findings.
