Presents the findings of an investigation into a workplace fatality in the mining and petroleum development sector that occurred in July 2012. A machinist was using a lathe to fabricate a spacer needed for changing out struts on haul trucks when his glove and coveralls become entangled in the rotating spacer. The machinist was taken to the hospital in Fort McMurray and later transported to hospital in Edmonton, where he later died. OHS administrative penalties can be imposed against any parties regulated by OHS legislation – workers, contractors, suppliers, prime contractors and employers. These penalties aim to encourage improved compliance with health and safety regulations, and are meant to work as a persuasive deterrent against those who demonstrate a chronic disregard for health and safety in the workplace. Administrative penalties came into effect October 1, 2013.
