S.P.F. no. 1. The certificate was revised as follows: the text expecting that the certificate be countersigned was removed. Insurers are reminded that the certificate is an approved form, and as such, its wording cannot be amended by insurers. Insurers are also reminded the certificate must bear the insurer's name as it appears on its license.
