This designated review focused on a six-month-old medically fragile child who died in parental care. The review team analyzed involvement of child intervention services by reviewing relevant documentation and interviewing staff and professionals working with the family. Findings and recommendations are intended to prevent or reduce the risk of future deaths and lead to better outcomes for children, youth and families.
