Description

Presents the findings of an investigation of workplace fatalities that took place in the manufacturing, packaging and processing sector in May 2019. Workers were testing a frac sand handling system for a sand bin, when the bin collapsed. Two workers were trapped in the sand and succumbed to their injuries.
