Description

Presents the findings of an investigation into a workplace fatality that occurred in February 2012. A worker received fatal blunt trauma injuries while testing a wood chipper that had been modified to chip plastic. Usage Considerations

Alberta Occupational Health and Safety (OHS) investigates serious incidents, including fatalities, which occur at work sites that fall under provincial OHS legislation. After a serious incident or fatality takes place at a work site, OHS officers investigate the work site to determine the causes and circumstances of the incident or fatality. OHS prepares an investigation report summarizing the incident and then conducts an enforcement action review of the investigation, to determine if the incident file should be submitted to Alberta Justice for consideration for prosecution. OHS publishes the fatality investigation report online only after investigations and all court proceedings, if any, are completed.
