Investigation report : worker fatally injured following collapse of equipment

Description

Presents the findings of an investigation into a workplace fatality in the construction and construction trades sector that occurred in September 2014. A worker was doing preparation work related to installing a rooftop makeup air unit. The unit's support curbing shifted, causing the unit to fall forward and onto the worker.
