Primary health care evaluation framework

Description

The Primary Health Care Evaluation Framework is intended to support ongoing assessment and evaluation of primary health care in the province and to contribute to continuing efforts to strengthen Alberta’s primary health care system. Although the Framework focuses on Primary Care Networks and the recently-introduced Family Care Clinics, it is intended to be applicable to all available primary health care services in Alberta. The Framework contains a series of discrete but associated components that can be used to evaluate unique aspects of Alberta Health’s service models. These components include a logic model, core evaluation questions, measurement concepts, and a description of the five key evaluation activities supported by the Framework.
