Description

Family Care Clinics (FCCs) are positioned to provide individual and family focused, comprehensive, quality, primary health care services across the lifespan, based on population health needs. The primary objectives are to: Increase and manage timely access to primary health care through same day access, extended hours of operation, on-call after hours and a triage function to provide direct access to most appropriate provider; Increase emphasis on health promotion, self-management, disease and injury prevention, screening and care of patients with chronic disease and complex needs; Utilize a collaborative interdisciplinary team approach working to full scope of practice; Improve coordination, continuity and integration of primary health care services.
