Presents the findings of an investigation into a workplace fatality in the transportation, communication and utilities sector that occurred in April 2015. A labourer working for a landscaping company was in a trench connecting a new sewer line from the municipality’s drainage system to a residential site under construction. While the labourer was in the trench, the south wall of the trench collapsed trapping and fatally injuring the labourer.

Presents the findings of an investigation into a workplace fatality in the transportation, communication and utilities sector that occurred in October 2015. A tow truck operator was loading a disabled truck onto a flatbed tow truck when a passenger car drove into the back of the tow truck, striking the tow truck operator. The force of the collision impact caused the loaded disabled truck to roll off of the tow truck bed and onto the passenger car, which in turn caused both vehicles to ignite. Both the tow truck operator and the driver of the passenger car were pronounced deceased at the scene.
