This publication presents the responses of Children and Family Services to recommendations made in mandatory reviews of child deaths undertaken by the Office of the Child and Youth Advocate. The Advocate is mandated to report on the death of young persons who had been identified as a child in need of intervention at the time of their death, or within two years of their death. These mandatory reviews are compiled and released twice a year (March and September).

The Advocate is mandated to report on the death of young persons who had been identified as a child in need of intervention at the time of their death, or within two years of their death. Contains the responses of the Ministry of Seniors, Community and Social Services to recommendations from the Office of the Child and Youth Advocate for mandatory reviews into child deaths. The Child and Youth Advocate Act provides the Advocate with the authority to conduct investigative reviews. The Advocate may investigate systemic issues arising from the death of a child who was receiving child intervention services within two years of their death. The Act requires each ministry to respond publicly within 75 days of the release of a report.

A steering committee made up of family members, service providers, advocates and government staff was created to present ideas and make recommendations on how to make the Persons with Developmental Disabilities (PDD) program better for the future. The committee presented 30 suggestions related to: people making their own decisions and being included in the community; government and the community making sure that people's needs are met; making sure that services meet people's needs and how people get services is fair; using technology to help people; and being flexible in how services are delivered.

In September 2021, the Office of the Child and Youth Advocate (OCYA) released the Strengthening Foundations report, which reviewed the circumstances of nine young people who died over a fourteen-month period in 2018 and 2019 and who had experienced similar challenges in service provision. The report provides two recommendations to address the limitations in assessments, information sharing and collaboration among service providers. This document contains the response of the Ministry of Community and Social Services to these recommendations.
