In December 2015, the Child and Youth Advocate released an investigative review which made two recommendations regarding trauma-informed care and case planning for placement and transitions. This publication provides the response of Ministry of Human Services to these two recommendations.

In 2013, a 17-year-old youth in parental care died by suicide following attendance at a rural hospital for suicidal ideation. The Child and Youth Advocate Act investigated the death and released a review which made three recommendations regarding collaboration, assessments and working with suicidal youth. This report provides the response of the Ministry of Human Services to these three recommendations.

This youth, now a young adult, receives supports and services to meet his needs as a result of the injury, which affected his motor skills, mobility, speech and memory. This document presents the Ministry response to the OCYA recommendations regarding assessments, healthy relationships and attachment, and working with suicidal youth.

In 2013, a staff at a group care facility went to awaken an eight-year-old girl in their care. She was breathing heavily and would not wake up, which was attributed to her prescribed sleep medication. Shortly thereafter, staff returned to wake her and found her unresponsive. Staff began CPR and called 911. Despite all efforts, the child was pronounced deceased at the hospital. The Office of the Chief Medical Examiner advised that the cause of death was trichloroethanol toxicity and the manner of death was undetermined. This report provides the Ministry response to the OCYA recommendations aboutabout three key areas related to practice and processes concerning services to vulnerable children in Alberta.

In 2013, a nine-year-old child living with her parents in her First Nations community was taken to hospital, where it was determined that she was suffering from septic shock as a result of severe dental decay. Inadequate housing, school attendance, neglect, parental addictions and chronic poverty were all noted concerns that were being addressed by several service delivery partners within the community. This report provides the Ministry response to the three recommendations made by the Office of the Child and Youth Advocate.

the Human Services Minister requested a review of the ICYHP group home. The scope of the review included examining staffing ratios, agency programming, resident safety, facility licensing and the contractual agreement between E4C and Edmonton Region CFS. Edmonton Region CFS conducted interviews and reviewed files and relevant documentation to assess the quality of care being provided by the group home, and to determine learnings for continuous improvement. The E4C agency was informed of the review from the onset.

The inquiry found that the cause of death was a stab wound to the abdomen and the manner of death was homicide. MC was in provincial government care at the time of his death. The report included two recommendations for the ministry. The ministry accepts both recommendations.
