In 2013, a 10-month-old infant in parental care passed away. The Office of the Chief Medical Examiner determined that the manner of death was accidental and the cause of death was drowning. The family was receiving services under assessment at the time of the death. Our thoughts are with this young child’s immediate and extended family and community.

In 2013, a staff at a group care facility went to awaken an eight-year-old girl in their care. She was breathing heavily and would not wake up, which was attributed to her prescribed sleep medication. Shortly thereafter, staff returned to wake her and found her unresponsive. Staff began CPR and called 911. Despite all efforts, the child was pronounced deceased at the hospital. The Office of the Chief Medical Examiner advised that the cause of death was trichloroethanol toxicity and the manner of death was undetermined. This report provides the Ministry response to the OCYA recommendations aboutabout three key areas related to practice and processes concerning services to vulnerable children in Alberta.

The Office of the Chief Medical Examiner determined that the manner and cause of death was undetermined. This report provides the Ministry response to the OCYA recommendations about two key areas related to practice and processes concerning services to vulnerable children in Alberta.

The inquiry found that the cause of death was a stab wound to the abdomen and the manner of death was homicide. MC was in provincial government care at the time of his death. The report included two recommendations for the ministry. The ministry accepts both recommendations.

The inquiry found that the cause of death was positional asphyxia and the manner of death was accidental. The child was in provincial government care at the time of death. The Fatality Inquiry report included two recommendations for the ministry. The recommendations are focused on policy revisions and engagement in best practices for the use of bassinets and safe sleep practices for children in provincial care.

The inquiry found that both the cause of death and the manner of death were undetermined. The child was in provincial government care at the time of death. The fatality inquiry report makes a total of nine recommendations for the Ministry, separated into two categories: recommendations for the prevention of similar deaths and recommendations to reduce stress.

The inquiry found this death was due to acute methadone toxicity, and the manner of the child’s death was accidental. The child was in provincial government care at the time of death and died while on a sleepover at a friend’s home.

The report from the Office of the Child and Youth Advocate made six recommendations about four key areas related to practice and processes with the child intervention system in Alberta. The Ministry of Human Services agrees with all six of the recommendations. The Ministry has a variety of activities and work underway which can be built upon to achieve these outcomes suggested in the recommendations.
