The all-party Ministerial Panel on Child Intervention was established to explore ways to improve Alberta’s child death review system and strengthen the intervention system. To help ensure transparency and accountability, panel meetings were held in public and on the record whenever possible. (Some meetings were held in private to better protect front line workers or families and to respect individual privacy.) Following every meeting, the panel provided a summary outlining what was discussed.

In 2014, a four-year-old little girl living under the guardianship of extended family in her First Nations community passed away in hospital after being brought in with life-threatening injuries. The child was not receiving intervention services at the time of her injuries. In November 2016, the Office of the Child and Youth Advocate released a report on its investigation into the death. This document presents the response of the Ministry of Children's Services to the OCYA report.

During Phase 1 of its work, the Panel provided detailed recommendations to streamline and strengthen the child death review process. Based on these recommendations, government passed Bill 18: The Child Protection and Accountability Act. In January 2014, the Minister of Human Services Minister announced a five-point plan for improving the child intervention system and an Implementation Oversight Committee (IOC) was appointed to oversee this work. Part of the IOC’s mandate was to prioritize responses to previous recommendations for improving the system, which came from a variety of sources, including the Office of the Child and Youth Advocate; the Alberta Child Intervention Review Panel; fatality inquiries; and the Child and Family Services Council for Quality Assurance. This report presents an independent assessment of the implementation status of some 67 previous recommendations, as identified by the IOC.
