Contains the responses of the Ministry of Seniors, Community and Social Services to recommendations from the Office of the Child and Youth Advocate for mandatory reviews into child deaths. The Child and Youth Advocate Act provides the Advocate with the authority to conduct investigative reviews. The Advocate may investigate systemic issues arising from the death of a child who was receiving child intervention services within two years of their death. The Act requires each ministry to respond publicly within 75 days of the release of a report.

Annual reports of the Family Support for Children with Disabilities Provincial Parent Advisory Committee.The committee provides input and feedback on policies, procedures and services as they affect families of children with disabilities to ensure legislation continues to have a positive impact on families and their children. The Advisory Committee also provides systemic advocacy for families of children with disabilities and assists in the development of strategies to increase public awareness on the challenges and opportunities faced by families of children with disabilities.

From October 2020 through May 2021, the Ministry of Community and Social Services engaged stakeholders as part of a review of the Family Support for Children with Disabilities program. This report summarizes the main themes of the feedback provided throughout the engagement. Participants shared thoughts on what they valued most about the program, the challenges they faced and ideas on how FSCD should be improved.

A steering committee made up of family members, service providers, advocates and government staff was created to present ideas and make recommendations on how to make the Persons with Developmental Disabilities (PDD) program better for the future. The committee presented 30 suggestions related to: people making their own decisions and being included in the community; government and the community making sure that people's needs are met; making sure that services meet people's needs and how people get services is fair; using technology to help people; and being flexible in how services are delivered.

In September 2021, the Office of the Child and Youth Advocate (OCYA) released the Strengthening Foundations report, which reviewed the circumstances of nine young people who died over a fourteen-month period in 2018 and 2019 and who had experienced similar challenges in service provision. The report provides two recommendations to address the limitations in assessments, information sharing and collaboration among service providers. This document contains the response of the Ministry of Community and Social Services to these recommendations.

The child was in an out-of-home placement through an agreement with the Family Support for Children with Disabilities (FSCD) program, but was visiting his father and stepmother at the time of death. The Ministry of Human Services has carefully reviewed the report and this document captures the ministry's response to the recommendation made in the report.

The Ministry of Human Services has carefully reviewed and considered the recommendations of the fatality inquiry report released on November 2, 2012, into the December 2006 death of Samantha Martin. The inquiry found this was a medical death due to natural causes, following the child’s return to parental care. Whenever possible, the ministry takes into account reviews of incidents involving children who were recently in care when identifying learnings that could lead to better services for children currently in the system.
