The Ministry of Children’s Services has carefully reviewed the Fatality Inquiry Report into the July 31, 2012 death of seven-year-old J.A.C., released on October 20, 2016. The inquiry found that the cause of death was drowning and the manner of death was accidental. The child was in provincial government care at the time of death.

The inquiry found that the cause of death was cranial trauma with a large blood clot pressing on the surface of the brain and the manner of death was homicidal. The child was in provincial government care at the time of death.

In 2013, a 10-month-old infant in parental care passed away. The Office of the Chief Medical Examiner determined that the manner of death was accidental and the cause of death was drowning. The family was receiving services under assessment at the time of the death. Our thoughts are with this young child’s immediate and extended family and community.

The Child and Youth Advocate Act investigated the death and released a review which made three recommendations regarding collaboration, assessments and working with suicidal youth. This document presents the Ministry response to the OCYA recommendations regarding assessments, healthy relationships and attachment, and working with suicidal youth.

In 2013, a staff at a group care facility went to awaken an eight-year-old girl in their care. She was breathing heavily and would not wake up, which was attributed to her prescribed sleep medication. Shortly thereafter, staff returned to wake her and found her unresponsive. Staff began CPR and called 911. Despite all efforts, the child was pronounced deceased at the hospital. The Office of the Chief Medical Examiner advised that the cause of death was trichloroethanol toxicity and the manner of death was undetermined. This report provides the Ministry response to the OCYA recommendations aboutabout three key areas related to practice and processes concerning services to vulnerable children in Alberta.

In 2013, a nine-year-old child living with her parents in her First Nations community was taken to hospital, where it was determined that she was suffering from septic shock as a result of severe dental decay. Inadequate housing, school attendance, neglect, parental addictions and chronic poverty were all noted concerns that were being addressed by several service delivery partners within the community. This report provides the Ministry response to the three recommendations made by the Office of the Child and Youth Advocate.
