Description

In 2009, the World Health Organization declared the first influenza pandemic in 41 years. The new strain was named influenza A(H1N1)pdm09. In 2010, the Minister of Alberta Health and Wellness (AHW) asked the Health Quality Council of Alberta to conduct a comprehensive review of the Alberta response to A(H1N1)pdm09. As part of this review, it was noted that Alberta had a higher mortality rate for than other provinces, but provincial differences in surveillance methods made inter-provincial comparisons difficult to interpret. In response to questions raised through the review, AHW committed to undertake a review of reported deaths to determine the role A(H1N1)pdm09 played in the death. This report presents the findings of the review.

The purpose of this report is to provide an overview of the impact on Alberta's health care system and its response to the Pandemic (H1N1) 2009 virus (pH1N1). With the first and second wave of pH1N1 complete, it is possible to do a quick assessment of the magnitude of the threat it represented to Albertans and the success of the health care system response. By combining surveillance data with the expertise from both Alberta Health and Wellness and Alberta Health Services, the report takes a comprehensive view of the two pandemic waves and provides contextual information to guide a public health response if a third pandemic wave materializes.

Description

The influenza pandemic outbreak experienced in 2009 was recognized as an opportunity to review Alberta’s plans and preparedness and determine what could be improved in a future response to another pandemic or other public health emergency. The Health Quality Council of Alberta (HQCA) was asked to conduct a review of the Alberta response. The review focused on the activities of Alberta Health and Wellness, Alberta Health Services and Alberta Emergency Management Agency. The review revealed many good outcomes and commendable practices from Alberta’s response to the pandemic which should continue in future responses to public health emergencies. It also identified systemic shortfalls that limited Alberta’s ability to respond effectively. The report makes 18 recommendations to address these shortfalls.

Following the declaration by the World Health Organization of the spread of the H1N1 virus as a global pandemic in the first half of 2009, Alberta Health and Wellness contracted a consulting firm to assist in the development of a governance model to enable timely cross-ministry decision-making during a pandemic response. A number of stakeholders were interviewed and two governance models were reviewed. This report presents a recommended pandemic response governance model which is adapted from the strengths of the two models under review.

This report outlines the lessons learned from the response to H1N1 Influenza and contains recommendations for improvements to the response capability of Alberta Health and Wellness and the province as a whole. The report does not comment upon the merit of the public health system response itself, but rather provides recommendations for the improvement of the emergency management capability of Alberta Health and Wellness. The recommendations were based upon the findings of over 16 after-action reviews conducted with more than 70 individuals who were involved in Alberta’s response to the H1N1 Influenza virus.
