A 40-year old male haul truck operator lost control of heavy haul truck when it was hit from behind and pushed over the berm on the left side of the haul road and crashed. Presents the findings of an investigation into a workplace fatality in the mining and petroleum development sector that occurred in December, 2008. A 48-year old male worker, employed as a process technician, was attempting to cut through an ice castle (very large chunk of ice) with a steam line to remove it from the steam tracing supply valves.
