Presents the findings of an investigation into a workplace fatality in the mining and petroleum development sector that occurred in May, 2014. A black bear attacked an electrical and instrumentation technician walking through the laydown yard near Fort McMurray and dragged the worker into the nearby bush. On-site emergency services department personnel and the RCMP were called. The RCMP euthanized the bear and the worker was found deceased in the wooded area. The operator used a rented zoom boom to move a metal I-beam at an oil production site. The welder helper was on the right side of the zoom boom. The zoom boom operator lost sight of the welder helper during a gradual turn and the helper sustained fatal crush injuries. Two pilot truck operators were piloting a rig equipment move. When pulling out of a gas station, one of the operators became entangled in the back end of one of the vehicles and was dragged approximately 200 metres out onto the highway, where he was freed from the jeep, and subsequently died from traumatic injuries. A machinist was using a lathe to fabricate a spacer needed for changing out struts on haul trucks when his glove and coveralls become entangled in the rotating spacer. The machinist was taken to the hospital in Fort McMurray and later transported to hospital in Edmonton, where he later died.
