Presents the findings of an investigation into a workplace fatality in the construction and construction trades sector that occurred in April, 2007. A 33-year old male worker, and a 27-year old male worker, employed as an electrician and a scaffolder were putting the roof on a large oil storage tank when the roof came down on the workers.
