Contains the responses of the Ministry of Seniors, Community and Social Services to recommendations from the Office of the Child and Youth Advocate for mandatory reviews into child deaths. The Child and Youth Advocate Act provides the Advocate with the authority to conduct investigative reviews. The Advocate may investigate systemic issues arising from the death of a child who was receiving child intervention services within two years of their death. The Act requires each ministry to respond publicly within 75 days of the release of a report.

In September 2021, the Office of the Child and Youth Advocate (OCYA) released the Strengthening Foundations report, which reviewed the circumstances of nine young people who died over a fourteen-month period in 2018 and 2019 and who had experienced similar challenges in service provision. The report provides two recommendations to address the limitations in assessments, information sharing and collaboration among service providers. This document contains the response of the Ministry of Community and Social Services to these recommendations.

On June 5, 2019, the Office of the Child and Youth Advocate released a report titled Mandatory Reviews into Child Deaths April 1, 2018–September 30, 2018. The report made five recommendations, four of which were directed towards Children’s Services (CS), and one of which was directed towards CS and Community and Social Services (CSS). This document contains the responses of the ministries to these five recommendations.
