A skid steer operator was spreading gravel inside the garage of a new construction home. The operator inadvertently activated the joystick causing the bucket arms to move. The left arm of the skid steer contacted the operator’s head and neck causing fatal injuries. A driver was unloading street sweepings from a dump truck. The driver exited the truck to investigate why the box's tail gate did not open. The gate suddenly opened and material poured out and buried the driver. The driver was taken to hospital and died five days later as a result of his injuries.
