Description

The purpose of this manual is to describe the standards, rules and business processes related to identity management to be applied when registering clients prior to delivering health services to them. This manual is not intended to be used as or replace site specific procedure manuals that support the business processes and systems, but is intended to support the practice of identity management in a consistent manner across the Alberta health system. This document should be used by any individual within the Alberta health system who is registering a client for health services or accessing an already existing client record.

The Notifiable Disease Report (NDR) Manual describes the roles and responsibilities of public health reporters and of Alberta Health in the systematic collection, analysis, interpretation and dissemination of data to appropriate stakeholders. It provides clarity on the notifiable disease reporting requirements for investigators. It also provides guidance for the completion of the Report of Notifiable Disease (NDR) form. The NDR form is intended to be a surveillance tool that collects meaningful disease information in Alberta in a manner that allows it to be easily analyzed, interpreted, and disseminated. Use of the NDR manual will help to ensure and maintain consistency in gathering information and interpretation of the form fields.
