Presents the findings of an investigation into a workplace fatality in the transportation communication and utilities sector that occurred in December, 2009. A 36-year old male worker employed as a gravel truck driver lost control of a loaded gravel hauler and went over an embankment at the south entrance to a one-lane bridge crossing a creek. A 41-year old male worker, employed as a journeyman power lineman, received a fatal electric shock. A “digger derrick” was raised and came into contact with a live 8000 volt overhead power line. The worker, who was on the ground, came into contact with the truck and received a fatal electrical shock.
