In October 2014, a young man was a victim of homicide in his home community. The young man was not receiving child intervention services at the time of his death. However, he had received services within the two years preceding his death. In March 2017, the Office of the Child and Youth Advocate’ released a report on its investigation into the death. This document presents the response of the Ministry of Children's Services to the OCYA report.

In 2013, a 10-month-old infant in parental care passed away. The Office of the Chief Medical Examiner determined that the manner of death was accidental and the cause of death was drowning. The family was receiving services under assessment at the time of the death. Our thoughts are with this young child’s immediate and extended family and community.

On December 8, 2015, the Advocate released an investigative review entitled Two-year-old Teanna: Serious Injury. The report makes two recommendations regarding case planning for placement and transitions, and ongoing assessment of capacity for caregivers. in this response, the ministry accepts the recommendations. Policy and practice requirements are clear and specific regarding the screening of potential caregivers and expectations for ongoing support, monitoring and review of placements.

The Child and Youth Advocate Act investigated the death and released a review which made three recommendations regarding collaboration, assessments and working with suicidal youth. This youth, now a young adult, receives supports and services to meet his needs as a result of the injury, which affected his motor skills, mobility, speech and memory. This document presents the Ministry response to the OCYA recommendations regarding assessments, healthy relationships and attachment, and working with suicidal youth.

In 2013, a staff at a group care facility went to awaken an eight-year-old girl in their care. She was breathing heavily and would not wake up, which was attributed to her prescribed sleep medication. Shortly thereafter, staff returned to wake her and found her unresponsive. Staff began CPR and called 911. Despite all efforts, the child was pronounced deceased at the hospital. The Office of the Chief Medical Examiner advised that the cause of death was trichloroethanol toxicity and the manner of death was undetermined. This report provides the Ministry response to the OCYA recommendations aboutabout three key areas related to practice and processes concerning services to vulnerable children in Alberta.
