Presents the findings of an investigation into a workplace fatality in the wholesale and retail trade sector that occurred in December, 2015. A store clerk, working alone on the overnight shift at a convenience store, was robbed by three masked individuals. The clerk was shot by one of the masked individuals as they were leaving the store. Presents the findings of an investigation into a workplace fatality in the manufacturing and processing sector that occurred in February 2014. A worker was conducting sawmill clean-up and start-up preparations when he walked out of the building into an outdoor yard carrying a load of lumber over his shoulder. At the same time, a front-end loader was operating, in reverse, alongside the building. The worker was struck by the front-end loader which caused fatal head injuries to the worker. Presents the findings of an investigation into a workplace fatality in the transportation, communication and utilities sector that occurred in October 2015. A tow truck operator was loading a disabled truck onto a flatbed tow truck when a passenger car drove into the back of the tow truck, striking the tow truck operator. The force of the collision impact caused the loaded disabled truck to roll off of the tow truck bed and onto the passenger car, which in turn caused both vehicles to ignite. Both the tow truck operator and the driver of the passenger car were pronounced deceased at the scene.
