Description

Presents the findings of an investigation into a workplace fatality in the mining and petroleum development sector that occurred in October, 2013. The operator used a rented zoom boom to move a metal I-beam at an oil production site. The welder helper was on the right side of the zoom boom. The zoom boom operator lost sight of the welder helper during a gradual turn and the helper sustained fatal crush injuries. Presents the findings of an investigation into a workplace fatality in the construction and construction trades sector that occurred in June, 2013. Due to a starter relay problem with the grader, a grader operator proceeded to start the grader by shorting across the starter terminals while standing between the left rear tires, adjacent to the engine compartment. Description

Presents the findings of an investigation into a workplace fatality in the construction and construction trades sector that occurred in March, 2014. A heavy equipment operator not fit for duty during a night shift was taken to his truck by his supervisor who then went to a trouble call. The supervisor later noticed that the operator was not in his truck and after an unsuccessful search the RCMP was called. A RCMP dog team found the operator’s body approximately 100 meters away in the bush, frozen. An apprentice heavy duty mechanic was working to troubleshoot lights on a vibrating drum packer. The mechanic fell out of the packer while exiting. When he fell, the packer moved forward and the rear left tire ran over the mechanic, causing fatal injuries. The purpose of the focused inspections was to raise awareness of the known workplace hazards in this industry and to ensure compliance with the Occupational Health and Safety Act, Regulation and Code. The overall goal of the project was to enhance employers’ and workers’ awareness of their responsibilities under the legislation by identifying and controlling known hazards associated with their work.

A peace officer following up on a dog/land use complaint had an altercation with the tenant, who overcame peace officer and put him in the back of his patrol vehicle. The tenant transported the peace officer to a Calgary Police Service station where he was found to be unresponsive. The tenant was criminally charged by CPS.
