On December 8, 2015, the Advocate released an investigative review entitled Two-year-old Teanna: Serious Injury. The report makes two recommendations regarding case planning for placement and transitions, and ongoing assessment of capacity for caregivers. in this response, the ministry accepts the recommendations. Policy and practice requirements are clear and specific regarding the screening of potential caregivers and expectations for ongoing support, monitoring and review of placements.

In 2013, a 16-year-old residing in care sustained a brain injury after attempting suicide. This youth, now a young adult, receives supports and services to meet his needs as a result of the injury, which affected his motor skills, mobility, speech and memory. This document presents the Ministry response to the OCYA recommendations regarding assessments, healthy relationships and attachment, and working with suicidal youth.

In 2013, a staff at a group care facility went to awaken an eight-year-old girl in their care. She was breathing heavily and would not wake up, which was attributed to her prescribed sleep medication. Shortly thereafter, staff returned to wake her and found her unresponsive. Staff began CPR and called 911. Despite all efforts, the child was pronounced deceased at the hospital. The Office of the Chief Medical Examiner advised that the cause of death was trichloroethanol toxicity and the manner of death was undetermined. This report provides the Ministry response to the OCYA recommendations aboutabout three key areas related to practice and processes concerning services to vulnerable children in Alberta.

In 2013, a nine-year-old child living with her parents in her First Nations community was taken to hospital, where it was determined that she was suffering from septic shock as a result of severe dental decay. Inadequate housing, school attendance, neglect, parental addictions and chronic poverty were all noted concerns that were being addressed by several service delivery partners within the community. This report provides the Ministry response to the three recommendations made by the Office of the Child and Youth Advocate.

The Child and Youth Advocate Act provides the Advocate with the authority to investigate systemic issues arising from a serious injury or death of a child who was receiving child intervention services at the time of the injury or death. Under this mandate, the Advocate released the Investigative Review entitled “7‐Year‐Old Jack: Investigative Review” on January 21, 2014. The report makes five recommendations about five key areas related to practice and processes within Alberta’s child intervention system. This document presents the ministry response to the recommendations.

The report makes five recommendations for Human Services about the delivery of services for youth who are leaving Government care. The ministry acknowledges the intent of the Advocate’s recommendations. The report suggests that previous responses from government to address issues for youth in care have not been effective. A review of the Advocate’s previous recommendations suggests that the ministry has, in fact, made progress and followed through on many recommendations; however, we do recognize that new tools, guidelines and policies, while important, are only part of the solution to improving outcomes for youth transitioning to adulthood.
