An implementation oversight committee was appointed to prioritize responses to previous recommendations for improving the child intervention system. It made recommendations to the Minister of Human Services in two progress reports, in April and August of 2014. This summary looks at the status of the ministry's response to each recommendation as of December 2016.

The inquiry found this death was due to acute methadone toxicity, and the manner of the child’s death was accidental. The child was in provincial government care at the time of death and died while on a sleepover at a friend’s home.
