Presents the findings of an investigation into a workplace fatality in the mining and petroleum development sector that occurred in December, 2008. A 48-year old male worker, employed as a process technician, was attempting to cut through an ice castle (very large chunk of ice) with a steam line to remove it from the steam tracing supply valves.
