This publication presents the responses of Children's Services to recommendations made in mandatory reviews of child deaths undertaken by the Office of the Child and Youth Advocate. The Advocate is mandated to report on the death of young persons who had been identified as a child in need of intervention at the time of their death, or within two years of their death. These mandatory reviews are compiled and released twice a year (March and September).

This publication presents the responses of Children's Services to recommendations made in mandatory reviews of child deaths undertaken by the Office of the Child and Youth Advocate. The Advocate is mandated to report on the death of young persons who had been identified as a child in need of intervention at the time of their death, or within two years of their death. From April to December 2021, there were 36 deaths of children, youth and young adults receiving services from the ministry of Children's Services. The Minister of Children's Services directed the ministry to carry out a review of the cases to gain an understanding of the circumstances surrounding these deaths, to help identify improvements that could be made to better serve children, youth and families. This publication reports on the findings of three separate reviews undertaken by the ministry, and actions that will be taken by the ministry in light of these findings.

This designated review focused on the deaths of two young children who received child intervention (CI) services while in parental care. The review team gathered information and conducted an analysis of CI involvement by reviewing relevant documentation and research. The team also interviewed CI practitioners, community partners, and subject matter experts. An analysis of the information and review of research informed the findings and recommendations. Throughout the designated review, the review team sought to understand the context of the worksites and communities, the level of service provided, and the decisions made during CI involvement.
