Presents the findings of an investigation into a workplace fatality in the mining and petroleum development sector that occurred in December, 2015. A motor coach operator dropped off workers at a T-intersection of a refinery worksite. As the motor coach moved forward to complete a left turn at the intersection, a worker crossing the road was struck and caught underneath the vehicle, suffering fatal injuries.
