Description

This decision process document was created to assist service providers of narcotic transition services in determining when to complete and submit a critical incident or duty to notify form. The Mental Health Services Protection Act requires the reporting of critical incidents that have caused or threatened to cause serious injury or death. In addition, service providers have a duty to notify or report on situations that may prevent service providers from delivering their services and/or situations that may be injurious to the safety or health of the patient.
