A 29-year old male worker was installing siding on the gable end of a new two storey home. During the work process the worker fell from an improperly-installed pump jack scaffold system 6.3 meters to the ground. Workers were using a heavy-duty tow truck winch to pull a trailer out of a ditch. The cable broke, swung back and struck a 59-year-old male worker in the head.
