Note: Revoked November 1, 2020, as the requirements it lists exist in Occupational Health and Safety legislation (such as the requirement to control or eliminate hazards) or Employment Standards legislation (such as the requirement to obtain the written consent of the adolescent's parent or guardian), making the order redundant.

Workers were using a heavy-duty tow truck winch to pull a trailer out of a ditch. The cable broke, swung back and struck a 59-year-old male worker in the head.
