Presents the findings of an investigation into a workplace fatality in the business personal and professional services sector that occurred in April, 2009. A 34-year old female worker, employed as a landscaper, was close to two workers but not assisting with the work at the time of the incident. The two worker were in the process of falling a tree when the tree cracked and one of the workers shouted a warning. The tree fell and struck the female worker on the head.
