A floorhand was working on the rig floor of a drilling rig. A deflector plate from the rig's top drive fell, striking the floorhand in the face. Four workers were attempting to manually erect a wall on a house. The wall became too heavy for the workers and they decided to let the wall fall. Three of the workers were able to step clear of the falling wall, but the fourth was caught beneath the wall and incurred fatal injuries.
