Description

Presents the findings of an investigation of a workplace fatality that occurred in April 2015 in the public administration, education and health services sector. A driver was unloading street sweepings from a dump truck. The driver exited the truck to investigate why the box's tail gate did not open. The gate suddenly opened and material poured out and buried the driver. The driver was taken to hospital and died five days later as a result of his injuries. A labourer working for a landscaping company was in a trench connecting a new sewer line from the municipality’s drainage system to a residential site under construction. While the labourer was in the trench, the south wall of the trench collapsed trapping and fatally injuring the labourer.
