Presents the findings of an investigation into a workplace fatality in the agriculture and forestry sector that occurred in November 2017. A worker was in an excavation attempting to expose a water line when the excavation wall sloughed and engulfed the worker. The worker suffered fatal injuries.

Two gravel trucks in close proximity were being unloaded at the same time. A dump box from one of trucks tipped over and crushed the cab of the other truck, fatally injuring the driver inside the cab.

An abrasive blasting operator opened the top hatch of a truck-mounted hopper to check levels. The hopper was under pressure and the pressure release caused the worker to fall to the ground. The worker struck his head and died of his injuries a week later.

A skid steer operator was spreading gravel inside the garage of a new construction home. The operator inadvertently activated the joystick causing the bucket arms to move. The left arm of the skid steer contacted the operator’s head and neck causing fatal injuries.
