The Ministry of Children’s Services has carefully reviewed the Fatality Inquiry Report into the July 31, 2012 death of seven-year-old J.A.C., released on October 20, 2016. The inquiry found that the cause of death was drowning and the manner of death was accidental. The child was in provincial government care at the time of death.

The Ministry of Human Services has carefully reviewed the fatality inquiry report into the January 17, 2010 death of six-month-old PO, released on November 5, 2014. The inquiry found that the cause of death was positional asphyxia and the manner of death was accidental. The child was in provincial government care at the time of death. The Fatality Inquiry report included two recommendations for the ministry. The recommendations are focused on policy revisions and engagement in best practices for the use of bassinets and safe sleep practices for children in provincial care.

The inquiry found that both the cause of death and the manner of death were undetermined. The child was in provincial government care at the time of death. The fatality inquiry report makes a total of nine recommendations for the Ministry, separated into two categories: recommendations for the prevention of similar deaths and recommendations to reduce stress.

The inquiry found this death was due to acute methadone toxicity, and the manner of the child’s death was accidental. Description

The Ministry of Human Services has carefully reviewed the fatality inquiry report released on February 28, 2014 regarding the death of RJ on November 11, 2010. The inquiry found that this death was due to cardiac arrest and anoxic brain injury, resulting from traumatic injury to the cervical spine following a fall down a flight of stairs in his home. The manner of death was accidental. The fatality inquiry report made seven recommendations for preventing similar deaths. In addition to the fatality inquiry review, an internal investigation was conducted by the Central Region PDD Community Board in November 2010 and made eight recommendations for the service provider. The service provider has implemented all the recommendations.
